PacifiCare Personal SDHP^SM & Personal Select^SM Plans are Preferred Provider Plans (PPO). These are a mirror of their very popular SignatureFreedom Group Health Plans. They offer Major Medical coverage with "first dollar" 100% coverage from a "Self Directed Health Account (SDA)". This means that there is no deductible 100% coverage for office visits and testing done in a Doctor's office. Up to the maximum you have in you SDA. This amount is $250 quarterly, and is deposited by PacifiCare into your SDA. If the money is not used in any year it "rolls over" to the next year. The funds spent from your SDA also apply to your Major Medical deductible. The SignatureOptions plan does not have the Self Directed Account.

PacifiCare's Health Maintenance Organization (HMO) plans focus on quality of care - with prevention and wellness as key components. Each HMO member accesses health care through a contracted primary care physician (PCPs) he or she selects. The PCP gets to know a member's medical history and individual needs, and thus a healthy partnership is formed between the member, the provider and PacifiCare. A member's primary care physician also selects the specialists and hospital the member will use when necessary.
