Within the last five (5) years, have you (the applicant) had ANY diagnosis, treatment, or professional medical advice relating to any of the following medical conditions? If you have one of these conditions and you are an Oregon resident you automatically qualify for OMIP/FMIP coverage under the Medical Eligibility Criteria. A physician must verify and confirm the presence of any pre-existing condition(s).
