The Beneficial Rx plan is best for those looking for a higher level of benefits and a lower total out-ofpocket cost. The Beneficial Rx plan includes services that can be accessed before the deductible, including preventive care, pharmacy services, limited doctor’s office or urgent care center visits, and alternative care.

ODS Individual Dental Plan [More Info]
Looking for a dental plan to add to your ODS individual health benefit plan coverage? Our dental plan gives you the freedom to choose your own dental professional with no waiting period or deductible for preventive services.

Underwriting
Applicants must be approved by ODS underwriting. Effective dates are assigned by underwriting and are generally the first day of the month following underwriting approval.

Billing
Members have the option to pay monthly by electronic checking account deduction, monthly via billing statement subject to a $2.00 fee per month or quarterly via a billing statement. Premium must be collected and sent in with the application.
