Coverage is available if you live in Benton, Clackamas, Columbia, Hood River, Lane, Linn, Marion, Multnomah, Polk, Washington, or Yamhill County in Oregon. You will be able to purchase:

• Health Net Violet Option 1 for $99 per month, which includes Part D prescription drug coverage.
• Health Net Violet Option 2 for $0 per month, which includes Part D prescription drug coverage.

Coverage is available if you live in Douglas, Jackson or Josephine County in Oregon, you will be able to purchase:

• Health Net Violet Option 1 for $91 per month, which includes Part D prescription drug coverage. (Douglas, Jackson or Josephine Counties)
• Health Net Violet Option 2 for $0 per month, which includes Part D prescription drug coverage.

The Health Net Violet plans are Medicare Advantage Preferred Provider Organization (PPO) plan offering healthcare freedom of choice in using medical providers. Receiving care from network providers gives you a higher level of benefits, but these health insurance plans (designed for Seniors) allow you the freedom to go out of network at a higher cost to you if you choose.

The Health Net Ruby plan is a Health Maintenance Organization (HMO) medicare insurance plan. This health care plan is available in Clackamas, Multnomah and Washington Counties, OR. You can only use doctors that are part of the Health Net network.

The Aqua plan is similar without Medicare Part D prescription drug coverage; however, purchasing it without this coverage could subject you to a 1% per month penalty every month you delay. You can never get rid of this penalty if you later need prescription drug coverage. You are not even allowed to buy a separate Part D plan and still keep this coverage. Aqua is an ideal choice if you have other creditable drug coverage provided through the VA or an employer plan.
