Choose from these individual and family plans if you are self-employed, between jobs, or just looking for coverage for yourself and your family. Health Net offers you the flexibility to choose a different coverage option for each family member.

Coverage Options

Health Maintenance Organization (HMO)
An affordable plan that allows you to select a Primary Care Physician to coordinate all your medical care.

Preferred Provider Organization (PPO)
These flexible options let you go directly to any doctor in our network of physicians - without a referral.

Eligibility and Service Area

You may apply for coverage if you are under age 65 and are not pregnant at the time of application. Eligible dependents include your spouse, the unmarried son or daughter (under age 23) of you or your spouse, or any other person chiefly supported by you or your spouse and for whom you or your spouse is a court-appointed guardian. All family members joining this plan, with the exception of full-time students, must live in the plan's service area. The service area includes the following Oregon counties: Benton, Clackamas, Clatsop, Columbia, Crook, Deschutes, Hood River, Jackson, Jefferson, Josephine, Lane, Linn, Marion, Multnomah, Polk, Wasco, Washington and Yamhill.
