About FHIAP | Eligible Plans

The following plans have been approved by FHIAP. If you have been approved for a FHIAP subsidy, please choose one of the plans and contact the health insurance carrier for an application. When you have completed the insurance carrier's application, sign and attach the Certificate of Eligibility to the application, and send it to them for approval. The Certificate of Eligibility alerts the health insurance company that you have been approved to receive a FHIAP subsidy.

Kaiser Permanente:

OMIP:

• OMIP Plan 500 - Medical Eligibility $500 deductible
• OMIP Plan 750 - Medical Eligibility $750 deductible

PacificSource:

• Elect Preferred $500 deductible

Providence:

• Optimum Plan 1000 $1000 deductible

Regence:

• Evolve Plus $1,000 deductible
